PARTICIPANT WAIVER, RELEASE & AGREEMENT

(Victoria, Australia)

I acknowledge and understand that participation in activities conducted by Nexus Jiu-Jitsu & MMA, located at 24/54 Bakers Road, Coburg North, Victoria, including but not limited to Brazilian Jiu-Jitsu, MMA, Wrestling, Striking, Strength & Conditioning, and the use of training equipment, involves inherent and significant risks. These activities may take place during training sessions, sparring, grading, competition preparation, or organised competitions, whether held on-site or off-site. These activities may include takedowns, throws, grappling, wrestling, joint manipulation, choke holds, striking, punching, kicking, resistance training, and physical contact. I understand that participation may result in injury, including but not limited to bruising, strains, sprains, fractures, concussion, spinal injury, permanent disability, or, in rare cases, death.

I voluntarily and knowingly assume all risks, whether foreseeable or unforeseeable, associated with participation. To the fullest extent permitted by the laws of Victoria, Australia, I release, discharge, and hold harmless Nexus Jiu-Jitsu & MMA, its owners, directors, coaches, employees, contractors, and affiliates from any and all claims or liability arising from participation, except where liability cannot legally be excluded under Australian Consumer Law.

I confirm that I (or my child) am medically fit to participate and accept full responsibility for monitoring my or my child's health and physical condition. If the participant is under 18 years of age,

I acknowledge that, as the parent or legal guardian, I am responsible for disclosing any known medical conditions, injuries, allergies, or health considerations that may affect the child’s ability to train safely.

I understand that failure to disclose relevant medical information may increase the risk of injury. I acknowledge that Nexus Jiu-Jitsu & MMA does not provide medical advice, diagnosis, or supervision, and that coaches rely on information provided by the participant or the participant's parent or guardian.

Responsibility for determining fitness to train remains with the participant or the participant's parent or guardian at all times. I agree to follow all safety instructions, academy rules, and coaching directions at all times. If the participant is under 18 years of age, I confirm that I am the legal parent or guardian and consent to the participant's participation on the participant's behalf.
